The Cleveland County Passport Acceptance Facility is inside the Register of Deeds office.  Applying for and obtaining a passport is quicker & more convenient for our citizens than going outside of the county.  We accept Passport applications on behalf of the U.S. Department of State.  You can view the current wait times on the US Department of State website.

BY APPOINTMENT ONLY

Passport Applications require an appointment and appointments are available Tuesday through Friday from 9 AM to 12 PM and 1 PM to 4 PM.  Please call to schedule an appointment.       704-484-4780


WHAT TO BRING TO YOUR APPOINTMENT

All applicants, including children, should be present at the time of the appointment.

Children under 16 must have both parents listed on their birth certificate with them.  If one parent is unable to appear they will need to sign a DS-3053 Parental Consent form, have the form notarized and make a photocopy of both sides of their ID for the applying parent to bring. (If one of the parents does not come, or does not sign a Consent Form, you will need to bring a certified copy of the Court Order showing the applicant parent has sole legal custody).

If the child is 16-17, either a parent should accompany to sign OR parent should be listed as an emergency contact and send a check with their name to cover the fees.

Please have the following when you arrive for your appointment:

  • Two separate payments are required, one to the CC ROD (Cash or Mastercard or Visa w/chip) and one to the U.S. Department of State (Check or Money Order).
  • Proof of Citizenship, i.e., Certified Birth Certificate (with parents listed), or previous Passport that is expired more than 5 years, or Naturalization Certificate.
  • Valid Driver’s License with clear photo, or valid State ID and Social Security Card for applicants 16 or older.
  • Print the DS-11 application form and fill out the areas shaded in brown on the last two pages. You can also fill out a copy in our office if that is more convenient.  (Please print single-sided pages at 100%, fill out in black ink, and do not use white out.). Do not sign until instructed to do so by an agent.
  • A recent photo (there are very specific requirements listed online; photo services are available in our office for $15.00 and for renewal applications also).

Some situations require additional documentation such as adoptions, foreign births, name changes other than marriage, etc.).  Check with the passport agent before making the appointment.
